Our services are delivered through three departments: Transitional Care Coordination, Post Hospitalization Chronic Illness Management, Supportive Living & Aging In Place Services.
TRANSITIONAL CARE COORDINATION: We help family caregivers to find information, explore options, discover answers and solutions when a loved one is going from one level of care or place of care to another. A daughter who lives out of state might use transitional care coordination when she needs to know if it is time for her 85 years old mother to move to an assisted living community or if she wants to know how to find the right doctor or service provider.
CHRONIC ILLNESS MANAGEMENT: We help older adults with chronic health conditions to learn how to manage symptoms, stay home, stay out of the emergency room and prevent unnecessary hospital readmissions. Through our coordination, this service also helps health care providers to deliver the best quality of care possible under the circumstances at hand.
Supportive Living & Aging In Place Services. Here, our goal is to provide personalized care that helps the client to reach optimal wellness through medication compliance, adequate nutrition, hydration, sleep, mobility, exercise and laughter. We will bring assisted living services into the home to allow the client to age in place on their own terms regardless of any life limiting conditions.
