Advocare is an integrated team of Geriatric Care Managers (GCMs) and care professionals including Nurses, Social Workers, and Life Care Planning professionals.

We help families to navigate the health & elder systems, using a coordinated approach called Concentric Circles of Care™.

Within the Outer Circle Care Managers serve as a quarterback for care in four distinct service areas:

1.Medical Care Management - Who would you want at your side during a crisis? Our Nurse Advocates accompany to key doctor visits, manage medications and monitor health status. They know the local resources - physicians, hospitals, rehab facilities, and other key providers, on call 24/7.

2.Transition Care Management - Things have changed for a parent, spouse, or loved one after years of flowing along smoothly. Whether it's a change in cognitive or medical status, you need to consider life transition options, such as choosing an assisted living community or a skilled nursing facility.

3.Life Care Planning - Helps answer the question, "Who pays for all of this?" Life Care Planning synchronizes the care plan with a financial plan, looking at insurance and entitlements such as veteran benefits and Medicaid.

4.Home Care Management - Caregivers & Care Managers collaborating to optimize care.

Within the Inner Circle Caregivers such as Home Health Aides and Certified Nursing Assistants assist with personal activities of daily living such as bathing, dressing, and standby support as well as shopping, food preparation, and light housekeeping.

Advocare's Home Care Management services match background-screened quality Caregivers to the precise needs of the client from our provider network of licensed agencies. The cost for caregivers in South Florida generally ranges from $15 to $20 per hour, with discounts for live-in. Our program adds a light layer of Home Care Management to ensure you have an advocate to help you access all needed care resources.
